Liability & Medical Waiver and Photo Release
I certify that I am the adult parent or guardian of child(ren) above, and I consent to his/her participation in activities at Lakeview Covenant Church in Northbrook. I understand and acknowledge that I am fully aware of and assume risk. I understand that Lakeview Covenant Church shall have no responsibility to pay for medical treatment and related costs if said minor child is injured. Knowing the minimal risks, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s participation in Lakeview Covenant Church’s VBS program. To the fullest extent allowed by law, I hold harmless and agree to indemnify Lakeview Church, its officers, directors, faculty, staff, volunteers, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or related to said minor child’s participation in Lakeview Covenant Church children’s programming. Additionally, I grant permission to Lakeview Covenant Church to use photographs and/or videos of my child taken during VBS for church ministry purposes, including print and digital media.
The registration fee is $20 per child (max $40 per family). You may pay via Zelle (lakeviewccfinance@gmail.com), give cash or a check to Eun Young Lee, or place it in the offering box. For all methods, please include a memo with "VBS" and your child's name, then text Eun Young Lee (817-713-6693) to confirm your payment.
